Enrolment Forms

 

If you are considering enrolling your child at Mount Lawley Senior High School, you will need to complete an Application for Enrolment Form (below).

The Admission Form needs to be brought to the school with the following documents:

• Original Full Birth Certificate.
• Passport if child born overseas.
• Relevant Visa documentation must also be provided.
• Access Restriction Documentation (if applicable).
• Four pieces of documentary proof of Residential address.
  • One must be a Lease Agreement from a recognised real estate agent of at least 12 months (if renting) or evidence of home ownership.
  • Three others (eg. Synergy, Alinta Gas or Telstra accounts, Centrelink Card, Home internet, Home insurance)
• Copy of Year 5 Semester 2 report
• Copy of Year 5 NAPLAN report
• ACIR Immunisation statement from Medicare - See link below

The application cannot be processed until originals of all documentation requested have been sighted by one of our School Officers and copies attached to the application.
